Has it been registered it in your name all that time or did you buy it recently? If the latter, then you SHOULD have registered it in your name and SORNed it immediately on purchase!
If you didn't register it, it's no problem, write your name etc in the changes section, tick the box for change of tax class and enter historic and take it to your local post office with the relevant MOT and insurance documentation (insurance paperwork SHOULDN'T be needed these days, but some POs are behind the times) They will issue your free tax, give you a reciept for £0.00 and send the V5 off to DVLA to get it sorted. Even if, as seems likely, you have an old style V5, it still works!
